Introduction:

Upper Airway Cough Syndrome (UACS), formerly known as postnasal drip syndrome, is a common cause of chronic cough. It is associated with the sensation of mucus trickling from the posterior nasopharynx.

Etiology:

  • Allergic Rhinitis: Reaction to allergens such as pollen, dust, or pet dander.
  • Non-Allergic Rhinitis: Triggered by irritants like smoke or strong odors, or changes in weather.
  • Infections: Acute or chronic sinusitis can lead to UACS.
  • Anatomical Variations: Deviated septum or nasal polyps may contribute to the condition.

Clinical Features:

  • Chronic cough, often described as worse at night or in the early morning.
  • A sensation of mucus dripping down the back of the throat.
  • Frequent throat clearing.
  • Nasal congestion and/or runny nose.
  • Sneezing and itchy throat in cases of allergic rhinitis.

Diagnosis:

  • Clinical Assessment: History of symptoms and physical examination, including nasal endoscopy.
  • Allergy Testing: Skin prick tests or specific IgE blood tests to identify allergens.
  • Imaging: Sinus X-rays or CT scans to detect sinusitis or anatomical abnormalities.
  • Rhinoscopy: Direct visualization of the nasal passages and pharynx.

Management:

  • Nasal Steroids: First-line treatment to reduce inflammation and mucus production.
  • Antihistamines: Especially if allergies are identified as a contributing factor.
  • Decongestants: To relieve nasal congestion, but should not be used long-term.
  • Nasal Irrigation: Saline sprays or neti pots to clear mucus.
  • Avoidance of Triggers: For allergic rhinitis, avoiding known allergens is crucial.
  • Surgery: In cases of anatomical obstruction or chronic sinusitis not responsive to medical therapy.

Prognosis:

  • With proper management, symptoms can often be well controlled.
  • Chronic cases may require ongoing treatment to manage symptoms.

Complications:

  • Disruption of daily activities due to persistent cough.
  • Sleep disturbances from nocturnal symptoms.
  • Development of lower respiratory tract symptoms if left untreated.

When to Seek Medical Attention:

  • If symptoms persist despite over-the-counter treatments.
  • Presence of additional symptoms such as fever, wheezing, or blood in the mucus.

Conclusion:

UACS is a common and treatable cause of chronic cough. Management includes identifying and treating underlying causes, symptom relief, and, in some cases, surgical interventions. Regular follow-up is important to adjust treatment as needed.
