Introduction:

A tyloma, also known as a callus, is an area of thickened skin that forms as a response to repeated friction, pressure, or other irritation. Tylomas are commonly found on the feet due to the pressure of walking or poorly fitting shoes, but they can occur on any part of the body that is subjected to ongoing rubbing.

Epidemiology:

  • Prevalence: Common in adults and individuals who are physically active or whose activities involve repetitive motions.
  • Risk Factors: Wearing ill-fitting shoes, manual labor, sports, and certain foot deformities.

Pathophysiology:

  • Skin Response: The skin reacts to chronic pressure or friction by hyperproliferating keratinocytes, leading to a thickened stratum corneum (the outermost layer of the skin).

Clinical Manifestations:

  • Thickened Skin: The most obvious sign is a thick, rough area of skin.
  • Hardened Raised Bumps: Can be tender or painless.
  • Flaky, Dry, or Waxy Skin

Diagnosis:

  • Physical Examination: Visual and tactile inspection of the affected area.
  • Dermatoscopy: May be used to rule out other skin lesions.
  • Biopsy: Rarely required unless there is suspicion of an underlying lesion.

Management:

  • Protective Padding: To redistribute pressure.
  • Proper Footwear: Shoes that fit well and offer good support.
  • Orthotic Devices: Custom-made insoles to correct foot posture.
  • Keratolytic Agents: Creams containing salicylic acid or urea to soften calluses.
  • Mechanical Debridement: Regular paring down of the thickened skin by a podiatrist.
  • Surgery: In rare cases, surgical intervention may be necessary to correct underlying structural problems.

Complications:

  • Pain: If left untreated, tylomas can become painful, especially when they are on weight-bearing areas of the foot.
  • Infection: Breakdown of overlying skin can lead to infection.
  • Ulceration: Particularly in individuals with diabetes or peripheral vascular disease.

Prevention:

  • Proper Footwear: Wearing shoes that fit correctly and provide adequate cushioning.
  • Foot Care: Regular use of pumice stones or foot files to maintain callus size.
  • Moisturizing Creams: To keep the skin supple and prevent cracking.

Prognosis:

  • Outlook: Generally good with proper treatment and preventive measures.
  • Recurrence: Likely if the precipitating factors are not addressed.

Tylomas are a common condition resulting from repeated pressure or friction. While they are typically not serious, they can cause discomfort and lead to complications if not managed properly. Preventive care and proper footwear are key to managing tylomas effectively.
