Introduction:

Tropical Pulmonary Eosinophilia (TPE) is a rare but serious hypersensitivity reaction to filarial parasites commonly found in tropical and subtropical regions. It is considered a variant of Loeffler's syndrome, which is characterized by pulmonary infiltrates and eosinophilia. TPE primarily affects the lungs, causing respiratory symptoms and can be mistaken for other respiratory conditions.

Causes and Pathophysiology:

TPE is caused by the body's exaggerated immune response to filarial parasites, particularly Wuchereria bancrofti and Brugia malayi. These parasites are transmitted to humans through mosquito bites. The parasites' microfilariae enter the blood circulation and can trigger an intense immune reaction in certain individuals, leading to TPE.

The immune response results in the release of inflammatory mediators and cytokines, attracting eosinophils to the lungs. Eosinophils are a type of white blood cell that plays a role in allergic reactions and fighting parasites. In TPE, these eosinophils accumulate in the lung tissue, causing inflammation and damage.

Symptoms:

The symptoms of TPE can vary, but common manifestations include:

  • Cough: A persistent dry or productive cough is a frequent symptom.
  • Breathlessness: Patients may experience shortness of breath, especially during physical activity.
  • Wheezing: Wheezing and chest tightness can occur due to airway inflammation.
  • Night Sweats: Excessive sweating during sleep is another common symptom.
  • Weight Loss: Unintended weight loss may be present due to the chronic inflammatory process.

Diagnosis:

TPE is often challenging to diagnose due to its nonspecific symptoms and similarity to other respiratory conditions. The following diagnostic tests can aid in confirming TPE:

  • Blood Tests: A complete blood count (CBC) can reveal elevated eosinophil levels, indicating eosinophilia.
  • Serology: Serological tests can detect antibodies against filarial parasites.
  • Imaging: Chest X-rays or CT scans may show lung infiltrates and opacities, indicating lung inflammation.
  • Sputum Examination: Microscopic examination of sputum may reveal microfilariae in some cases.
  • Lung Function Tests: Pulmonary function tests can assess lung capacity and airflow.

Treatment:

The primary treatment for TPE involves antifilarial drugs, such as diethylcarbamazine (DEC) or ivermectin, which effectively kill the microfilariae and adult worms. Corticosteroids may also be prescribed to reduce inflammation and eosinophil levels in the lungs, providing symptomatic relief.

Prevention:

Preventing mosquito bites in endemic regions is essential to reduce the risk of acquiring filarial infections. Using mosquito repellents, wearing protective clothing, and sleeping under insecticide-treated bed nets are effective preventive measures.
