Procedure:

The triple-phase bone scan involves the following three phases:

1. Blood Flow Phase:

In this phase, a radioactive tracer (usually technetium-99m labeled methylene diphosphonate) is injected into the patient's bloodstream. The tracer is rapidly taken up by the bone tissue with active blood flow. Images are acquired immediately after injection, showing areas of increased blood flow to the bones.

2. Blood Pool Phase:

The second phase takes place shortly after the blood flow phase, usually within a few minutes to an hour. During this phase, the scanner captures images of the tracer as it accumulates in the soft tissues around the bone. This phase helps identify areas of increased blood pooling, which may indicate inflammation or infection.

3. Bone Phase:

The final phase occurs a few hours after the injection, typically two to four hours later. The scanner captures images of the tracer distribution in the bones. Areas of increased tracer uptake may indicate bone abnormalities, such as fractures, tumors, or other bone diseases.

Indications:

A triple-phase bone scan is helpful in the diagnosis and evaluation of various bone and joint conditions, including:

  • Bone infections (osteomyelitis) and septic arthritis.
  • Bone fractures, stress fractures, and nonunion.
  • Bone tumors, such as osteosarcoma or metastatic bone lesions.
  • Complex regional pain syndrome (CRPS).
  • Avascular necrosis (osteonecrosis) of bone.
  • Evaluation of joint prostheses for loosening or infection.
  • Unexplained bone pain or suspected bone pathology.

Advantages:

The triple-phase bone scan offers several advantages as a diagnostic tool:

  • Non-invasive: The procedure involves the injection of a radioactive tracer, making it minimally invasive and safe for most patients.

Despite its advantages, the triple-phase bone scan has some limitations:

  • Radiation exposure: Although the radiation dose is relatively low, it is essential to consider cumulative exposure in patients who require multiple imaging studies.
