This infection affects both men and women, but symptoms are more common in women. Lifecycle and Transmission:

  • Direct Transmission: The parasite is primarily transmitted through sexual contact with an infected individual.
  • Survival Outside Host: Trichomonas vaginalis can survive in moist environments for several hours, potentially leading to transmission through contaminated objects, although this is less common.

Clinical Manifestations:

In Women:

  • Vaginal Discharge: Yellow-green, frothy, and sometimes malodorous discharge is characteristic.
  • Vulvovaginal Irritation: Itching, burning, and redness can be present.
  • Dysuria and Dyspareunia: Pain during urination and intercourse are common complaints.

In Men:

  • Men are often asymptomatic but may experience:
  • Urethritis: Irritation or inflammation of the urethra, leading to burning sensation during urination.
  • Prostatitis: Inflammation of the prostate gland, occasionally.

Diagnosis:

  • Microscopic Examination: Direct visualization of the motile parasites in vaginal or urethral secretions.
  • Culture: Growing the parasite in specialized media, although this is less commonly used due to the availability of more rapid tests.
  • Nucleic Acid Amplification Tests (NAATs): These are the most sensitive tests and can be used on urine samples or genital swabs.

Treatment:

  • Metronidazole or Tinidazole: These are the drugs of choice, effective in eradicating the infection with a single dose or short course.
  • Avoiding Alcohol: Patients should abstain from alcohol during treatment and for 24-72 hours after completing treatment with these medications due to possible disulfiram-like reactions.
  • Partner Notification and Treatment: To prevent reinfection and further transmission, sexual partners should be informed and treated.

Prevention:

  • Safe Sexual Practices: Consistent use of condoms and dental dams can reduce the risk of transmission.
  • Regular STI Screening: Especially for those with multiple sexual partners or new relationships.
  • Education: Raising awareness about STIs and promoting sexual health.
