Introduction:

TORCH infections represent a group of vertically transmitted infections that can cause serious health issues in newborns if the mother contracts them during pregnancy. The acronym "TORCH" stands for Toxoplasmosis, Other (including syphilis, varicella-zoster, parvovirus B19), Rubella, Cytomegalovirus (CMV), and Herpes simplex virus (HSV). These infections can lead to a range of birth defects and even fetal death if not diagnosed and managed promptly.

Toxoplasmosis:

  • Caused by: Toxoplasma gondii, a protozoan parasite.
  • Transmission: Consuming undercooked meat, contact with infected cat feces, or mother-to-fetus during pregnancy.
  • Symptoms in Newborns: Jaundice, enlarged liver and spleen, pneumonia, seizures, and chorioretinitis.
  • Prevention: Avoiding undercooked meat, washing hands after handling soil or cat litter.

Other infections:

  • Syphilis: Caused by Treponema pallidum, it can lead to stillbirth, premature birth, or serious congenital disorders.
  • Varicella-Zoster Virus: The causative agent for chickenpox and shingles. In neonates, it can cause congenital varicella syndrome.
  • Parvovirus B19: Responsible for fifth disease in children. In pregnant women, it can lead to fetal anemia and hydrops fetalis.

Rubella (German Measles):

  • Caused by: Rubella virus.
  • Transmission: Respiratory droplets.
  • Symptoms in Newborns: Cataracts, deafness, heart defects, intellectual disabilities, and liver and spleen damage.
  • Prevention: MMR vaccine, which also protects against measles and mumps.

Cytomegalovirus (CMV):

  • Caused by: CMV, a type of herpesvirus.
  • Transmission: Direct contact with bodily fluids, organ transplants, blood transfusions, or vertical transmission during childbirth.
  • Symptoms in Newborns: Jaundice, rash, low birth weight, microcephaly, and seizures.
  • Prevention: Good hygiene, avoiding sharing food, drinks, or utensils with young children.

Herpes Simplex Virus (HSV):

  • Caused by: Herpes simplex virus types 1 and 2.
  • Transmission: Direct contact with herpes sores or by vertical transmission during childbirth.
  • Symptoms in Newborns: Skin, eye, and mouth sores, central nervous system infections, and disseminated herpes (affecting multiple organs).
  • Prevention: Antiviral treatment for pregnant women with HSV, cesarean delivery if active herpes sores are present during labor.

Conclusion:

TORCH infections, though diverse in their origins and manifestations, collectively pose a significant threat to fetal and neonatal health. Early detection and management in pregnant women, along with preventive measures, are crucial to reduce the risk of congenital infections and ensure the health of the newborn. Regular prenatal check-ups, awareness about these infections, and timely vaccinations can play a pivotal role in safeguarding both maternal and neonatal well-being.
