Introduction:

Tetralogy of Fallot (TOF) is one of the most common congenital heart defects. It's characterized by four distinct heart abnormalities that occur together. These defects affect the structure of the heart and cause oxygen-poor blood to flow out of the heart and into the rest of the body.

Anatomy and Pathophysiology:

TOF consists of four primary defects:

  • Ventricular Septal Defect (VSD): A hole in the wall between the heart's two lower chambers, the ventricles.
  • Pulmonary Stenosis: Narrowing of the pulmonary valve and the passage through which blood flows from the right ventricle to the pulmonary artery.
  • Right Ventricular Hypertrophy: Thickening of the muscular walls of the right ventricle, which occurs because the heart has to work harder to pump blood through the narrowed pulmonary valve.
  • Overriding Aorta: The aorta is positioned directly over the VSD, allowing oxygen-poor blood to flow into the aorta and out to the body.

Clinical Manifestations:

  • Cyanosis: A blue or purple tint to the skin, lips, and nails due to reduced oxygen in the blood.
  • Shortness of Breath: Especially during feeding or activity.
  • Clubbing: Enlargement or rounding of the fingertips.
  • "Tet Spells": Sudden episodes of deep blue skin, shortness of breath, or fainting. These can be triggered by crying, feeding, or defecation.
  • Poor Weight Gain: Difficulty in thriving due to the increased work of breathing and the heart working overtime.

Diagnostic Approaches:

  • Echocardiogram: Uses sound waves to produce detailed images of the heart's structure and function.
  • Electrocardiogram (ECG): Measures the electrical activity of the heart.
  • Chest X-ray: Can show the characteristic "boot-shaped" heart seen in TOF.
  • Cardiac MRI: Provides detailed images of the heart and surrounding structures.
  • Cardiac Catheterization: A procedure that gives detailed information about the structures inside the heart.

Treatment Modalities:

  • Surgery: The only effective treatment for TOF. The primary surgical approach is intracardiac repair, usually performed in the first year of life.
  • Temporary Procedures: For babies who are too small or ill to undergo full repair, a shunt might be placed to increase blood flow to the lungs.
  • Long-term Monitoring: Regular follow-ups with a cardiologist are essential, as complications can develop over time.

Long-Term Implications and Prognosis:

With early diagnosis and appropriate surgical intervention, most children with TOF can lead healthy lives. However, they often require lifelong medical care, including regular check-ups, medications, and sometimes additional surgeries or procedures.
