Clinical Manifestations:

Postpartum thyroiditis typically progresses in two phases:

Thyrotoxic Phase: Occurring within the first 1-6 months postpartum, symptoms mimic hyperthyroidism:

  • Fatigue
  • Palpitations
  • Irritability
  • Heat intolerance
  • Weight loss

Hypothyroid Phase: Starting from the initial weeks to several months postpartum, symptoms resemble hypothyroidism:

  • Fatigue
  • Weight gain
  • Cold intolerance
  • Dry skin
  • Depression

Many women eventually revert to a euthyroid state, but some might develop permanent hypothyroidism.

Diagnostic Paradigms:

  • Blood Tests: During the thyrotoxic phase, low TSH and elevated Free T4 and T3 levels are typical. Thyroid antibodies, like anti-TPO, might be present.
  • Radioactive Iodine Uptake Test: This test can differentiate postpartum thyroiditis from Graves' disease. Postpartum thyroiditis will show low uptake, while Graves' disease will show increased uptake.

Management and Treatment:

  • Symptomatic Treatment: Beta-blockers can be used during the thyrotoxic phase to manage symptoms.
  • Thyroid Hormone Replacement: If the hypothyroid phase is prolonged or if the individual becomes permanently hypothyroid, levothyroxine may be prescribed.
  • Regular Monitoring: Periodic thyroid function tests are essential to track the course of the condition and adjust treatments accordingly.
