Introduction: 

NSTEMI, which stands for Non-ST-Segment Elevation Myocardial Infarction, is a type of heart attack. Unlike STEMI (ST-Segment Elevation Myocardial Infarction), NSTEMI doesn't always show specific changes on an electrocardiogram (ECG). In this article, we will explore the causes, symptoms, diagnosis, and treatment of NSTEMI.

Causes:

  • Coronary Artery Disease (CAD): NSTEMI is primarily caused by CAD, where the coronary arteries become narrowed or blocked by plaque buildup. This reduces blood flow to the heart muscle.
  • Blood Clot Formation: In NSTEMI, a blood clot can partially block a coronary artery. This partial blockage can lead to reduced blood supply and damage to the heart muscle.

Symptoms:

  • Chest Pain: The most common symptom is chest pain or discomfort. It is often described as a squeezing, pressure-like pain in the chest that can radiate to the arms, neck, jaw, or back.
  • Shortness of Breath: NSTEMI may cause sudden shortness of breath, especially during physical activity or at rest.
  • Fatigue: Unexplained fatigue and weakness.
  • Nausea and Vomiting: Some individuals may experience nausea or vomiting.
  • Sweating: Profuse sweating, often described as "breaking out in a cold sweat."

Diagnosis:

  • Electrocardiogram (ECG): While NSTEMI may not show the classic ST-segment elevation seen in STEMI, ECG changes such as ST-segment depression or T-wave inversion may be present.
  • Blood Tests: Cardiac biomarkers like troponin are measured. Elevated troponin levels indicate heart muscle damage.
  • Coronary Angiography: A coronary angiogram is performed to visualize the coronary arteries and identify blockages.

Treatment:

  • Medications: Medications are often prescribed to manage NSTEMI, including antiplatelet drugs, anticoagulants, beta-blockers, and nitroglycerin to relieve chest pain.
  • Angioplasty and Stent Placement: In some cases, a procedure called percutaneous coronary intervention (PCI) is performed to open blocked arteries and place stents.
  • Coronary Artery Bypass Grafting (CABG): In severe cases, CABG surgery may be necessary to bypass blocked coronary arteries.

Prognosis: 

The prognosis for NSTEMI varies depending on factors like the extent of heart muscle damage, the presence of underlying conditions, and the timeliness of treatment. With prompt medical intervention and lifestyle changes, many people can lead healthy lives after an NSTEMI.
