Introduction:

Non-ST-segment elevation myocardial infarction (NSTEMI) is a type of heart attack that is characterized by reduced blood flow to the heart muscle. Chest pain, also known as angina, is a common symptom of NSTEMI. This article provides insights into NSTEMI-related chest pain, its causes, symptoms, diagnosis, and management.

Understanding NSTEMI and Chest Pain:

NSTEMI occurs when there is a partial blockage in one or more coronary arteries, leading to decreased blood flow to the heart muscle. This can result from the rupture of an atherosclerotic plaque and the formation of a blood clot in the coronary artery. Chest pain, or angina, is a hallmark symptom of NSTEMI and is often characterized by:

  • Location: Chest pain is typically felt behind the breastbone (sternum) but can also radiate to the arms, neck, jaw, back, or stomach.
  • Quality: The pain is often described as a pressure, squeezing, tightness, or burning sensation.
  • Duration: Chest pain in NSTEMI is usually prolonged, lasting for more than 10-20 minutes, and is not relieved by rest or nitroglycerin, which is different from stable angina.
  • Associated Symptoms: Nausea, vomiting, sweating, shortness of breath, and lightheadedness may accompany chest pain during an NSTEMI.

Diagnosis and Evaluation:

Diagnosing NSTEMI involves a combination of clinical evaluation and diagnostic tests, including:

  • Electrocardiogram (ECG or EKG): This test records the electrical activity of the heart and can detect changes indicative of a heart attack.
  • Blood Tests: Elevated cardiac biomarkers, such as troponin and creatine kinase-MB (CK-MB), are often present in the blood during an NSTEMI.
  • Coronary Angiography: This invasive procedure involves injecting contrast dye into the coronary arteries to visualize blockages or narrowed blood vessels.

Management and Treatment:

Immediate management of NSTEMI focuses on restoring blood flow to the heart muscle and preventing further damage. Treatment options include:

  • Medications: Antiplatelet drugs, anticoagulants, and medications to reduce chest pain and improve blood flow are commonly prescribed.
  • Coronary Angioplasty: In some cases, percutaneous coronary intervention (PCI) may be performed to open blocked or narrowed coronary arteries and place stents to keep them open.
  • Coronary Artery Bypass Grafting (CABG): This surgical procedure may be considered if multiple coronary arteries are blocked or if PCI is not suitable.
  • Lifestyle Changes: Adopting a heart-healthy lifestyle, including a balanced diet, regular exercise, smoking cessation, and stress management, is crucial for long-term management.

Conclusion:

NSTEMI is a type of heart attack characterized by reduced blood flow to the heart muscle. Chest pain is a hallmark symptom of NSTEMI and often requires immediate medical attention. Early diagnosis and prompt intervention, including medications and interventions like angioplasty, can significantly improve outcomes. It is essential for individuals to recognize the symptoms of NSTEMI, especially chest pain, and seek timely medical care.
