Introduction:

Müller's Muscle-Conjunctival Resection (MMCR) is a surgical procedure commonly used for the correction of blepharoptosis, a condition characterized by drooping of the upper eyelid margin. MMCR is particularly effective in cases of mild to moderate ptosis, improving both visual function and cosmetic appearance.

Procedure:

During the MMCR procedure, the surgeon makes a small incision along the natural eyelid crease, usually about 5-8 mm above the eyelid margin. The Müller's muscle and conjunctiva are then carefully dissected from the underlying tissues. A small segment of Müller's muscle and conjunctiva is excised, effectively shortening the muscle. The remaining tissues are reattached to the tarsal plate (the dense connective tissue of the upper eyelid). This reattachment elevates the eyelid and improves its positioning.

Indications:

MMCR is suitable for patients with mild to moderate blepharoptosis, where the levator muscle (the muscle responsible for lifting the eyelid) functions adequately, but its action is hindered by an elongated or weakened Müller's muscle. Patients with congenital ptosis or involutional ptosis caused by aging are often good candidates for this procedure.

  • Cold Compresses: Applying cold compresses can help reduce swelling and bruising.
  • Eye Lubrication: Using lubricating eye drops can alleviate dryness and discomfort.
  • Avoiding Vigorous Activities: Patients should avoid strenuous activities that might strain the surgical site.

Outcomes:

MMCR typically results in improved eyelid positioning and enhanced aesthetics. It may also address functional issues caused by ptosis, such as visual field obstruction. However, as with any surgical procedure, there are potential risks, including infection, scarring, asymmetry, and recurrence of ptosis.
