Introduction:

Marginal keratitis is a type of corneal inflammation that affects the peripheral or marginal area of the cornea. It is characterized by inflammatory infiltrates and ulcerations in the limbal region, which is the area where the cornea meets the white of the eye (sclera). This condition can cause discomfort, redness, and vision disturbances. In this article, we will explore the causes, symptoms, diagnosis, and treatment of marginal keratitis.

Causes:

  • Microbial Infections: Bacterial, viral, or fungal infections can lead to marginal keratitis. Common pathogens include Staphylococcus aureus, Herpes simplex virus, and fungi like Fusarium and Aspergillus.
  • Immune-Mediated Disorders: Some cases of marginal keratitis are associated with immune-mediated diseases such as rheumatoid arthritis, systemic lupus erythematosus (SLE), or Mooren's ulcer.
  • Contact Lens Wear: Improper use of contact lenses, poor hygiene, or wearing them for extended periods can lead to marginal keratitis, especially if there is corneal trauma or infection.
  • Allergies: Allergic reactions to environmental factors, such as pollen or contact lens solutions, can trigger inflammation in the corneal periphery.
  • Dry Eye Syndrome: Chronic dryness and irritation of the eyes can make the cornea more susceptible to inflammation, including marginal keratitis.

Symptoms:

  • Eye redness and irritation
  • Foreign body sensation or grittiness in the eye
  • Blurred vision
  • Excessive tearing
  • Photophobia (sensitivity to light)
  • White or grayish infiltrates in the limbal area

Diagnosis:

A comprehensive eye examination is crucial for diagnosing marginal keratitis. The ophthalmologist will:

  • Examine the eyes with a slit lamp to assess the cornea and limbal region.
  • Evaluate the patient's medical history to identify potential underlying causes or risk factors.
  • May perform corneal scrapings or cultures to determine if an infection is present.

Treatment:

The treatment approach for marginal keratitis depends on the underlying cause and severity of the condition. It may include:

  • Topical Steroids: To reduce inflammation and alleviate symptoms.
  • Antibiotics or Antiviral Medications: If the cause is bacterial or viral in nature.
  • Antifungal Medications: If a fungal infection is suspected.
  • Artificial Tears: To lubricate the eyes and alleviate dryness.
  • Allergy Medications: For allergic marginal keratitis.
  • Cycloplegic Drops: To relieve pain and photophobia.
  • Contact Lens Management: If improper lens use is the cause, proper cleaning, and usage guidance.

Prevention:

Prevention of marginal keratitis includes practicing good eye hygiene, proper contact lens care, and avoiding triggers that may cause allergic reactions. Regular eye check-ups are essential to monitor eye health and identify any issues early.

Conclusion:

Marginal keratitis is a condition characterized by inflammation and ulceration in the limbal area of the cornea. It can be caused by infections, immune-mediated disorders, allergies, or improper contact lens use. Timely diagnosis and appropriate treatment are crucial to prevent complications and preserve vision.
