Introduction:

Fluorescein angiography (FA) is a diagnostic imaging technique commonly used in ophthalmology to visualize and assess the blood flow within the retinal and choroidal vasculature. It involves the intravenous injection of fluorescein dye and the capture of sequential images as the dye travels through the blood vessels in the eye.

Procedure:

During the procedure, a small amount of fluorescein dye is injected into a vein in the arm. The dye rapidly circulates through the bloodstream and reaches the blood vessels of the retina and choroid within seconds. A specialized camera equipped with filters for detecting the fluorescence is used to capture images of the retina at different time points after the dye injection.

Indications:

Fluorescein angiography is a valuable tool for diagnosing and monitoring various ocular conditions, including:

  • Age-Related Macular Degeneration (AMD): FA can identify abnormal blood vessel growth (choroidal neovascularization) that occurs in some forms of AMD.
  • Diabetic Retinopathy: FA helps visualize areas of leakage and ischemia caused by damaged blood vessels in diabetic retinopathy.
  • Retinal Vascular Occlusions: It can detect blocked blood vessels and areas of reduced blood flow in the retina.
  • Macular Edema: FA can identify areas of fluid leakage in the macula, leading to swelling and vision loss.
  • Retinal Vein Occlusion (RVO): FA helps identify areas of vein blockage and the extent of retinal ischemia.
  • Choroiditis and Chorioretinitis: FA can visualize inflammation in the choroid and retina.

Procedure Interpretation:

The images obtained from FA are studied by ophthalmologists and retina specialists to assess the patterns of blood flow and detect abnormalities in the blood vessels. The presence of leakages, dye staining, and vessel changes can provide crucial information for diagnosis and treatment planning.

Side Effects and Risks:

Fluorescein angiography is generally considered safe. However, potential side effects and risks include:

  • Allergic Reactions: Some individuals may experience mild allergic reactions to the dye injection.
  • Nausea and Vomiting: Transient nausea and vomiting can occur in a small number of patients due to the dye injection.
  • Skin Discoloration: Temporary skin discoloration may be observed at the injection site.
  • Rare Adverse Reactions: Rarely, severe allergic reactions can occur, but they are very uncommon.

Conclusion:

Fluorescein angiography is a valuable diagnostic tool used to evaluate the vascular status of the retina and choroid. It plays a crucial role in diagnosing and managing various retinal and choroidal vascular conditions, helping ophthalmologists provide better care and treatment options for patients.
