It can decrease blood flow, preventing nourishment and oxygen from reaching nerve and muscle cells. The condition can be either acute or chronic, each with distinct causes and treatment approaches. Understanding the types, causes, symptoms, diagnostic procedures, and treatment options for compartment syndrome is crucial for preventing long-term damage.

Types of Compartment Syndrome:

  • Acute Compartment Syndrome: Usually develops after a severe injury. It is a medical emergency and requires immediate treatment.
  • Chronic Compartment Syndrome: Also known as exertional compartment syndrome, it is usually caused by athletic exertion.

Causes:

  • Acute: Often caused by a traumatic injury such as a fracture, severe bruise, or crush injury. Can also occur after surgery or reperfusion injury (blood flow returning to tissue after a period of ischemia or lack of oxygen).
  • Chronic: Caused by repetitive muscle use during activities like running or cycling. The muscles become too large for the compartments they are in, leading to pressure buildup.

Symptoms:

  • Severe pain that does not go away when you take painkillers or elevate the injured area.
  • Pain that seems greater than what would be expected for the severity of the injury.
  • Decreased sensation, tingling, or burning sensation in the skin.
  • Tightness and swelling in the muscle.
  • For acute cases, symptoms develop rapidly and can include paleness and coldness in the affected limb.

Diagnostic Evaluation:

  • Diagnosis is primarily clinical, based on symptoms and physical examination.
  • Compartment pressure measurement: A test to measure the pressure within the muscle compartments.
  • Imaging tests like MRI or CT scans may be used to assess the injury and plan treatment in some cases.

Treatment and Management:

  • Acute Compartment Syndrome: Requires immediate surgery called a fasciotomy to relieve pressure. Delay in treatment can lead to permanent muscle and nerve damage.
  • Chronic Compartment Syndrome: Often treated conservatively with rest and modification of activities. Physical therapy and orthotic devices may help. Surgery is an option if conservative measures fail.

Complications:

  • Untreated acute compartment syndrome can lead to permanent muscle and nerve damage, infection, and kidney failure.
  • Chronic compartment syndrome can limit athletic performance and activities.

Prevention and Patient Education:

  • Education about the importance of early treatment for acute injuries.
  • Athletes should be informed about the risks of overtraining and the importance of proper technique.
