Cirrhosis is a late stage of scarring (fibrosis) of the liver caused by many forms of liver diseases and conditions, such as hepatitis and chronic alcoholism. Each time the liver is injured, it tries to repair itself, leading to scar tissue. Over time, the accumulation of scar tissue impairs liver function. This article details the causes, symptoms, diagnostic procedures, and treatment options for cirrhosis.

Causes of Cirrhosis:

  • Chronic alcohol abuse is a leading cause.
  • Chronic viral infections, particularly hepatitis B and C.
  • Nonalcoholic fatty liver disease (NAFLD), including nonalcoholic steatohepatitis (NASH).
  • Autoimmune hepatitis, genetic disorders, bile duct diseases, and certain medications.

Progression and Symptoms:

  • Early stages of cirrhosis often have no symptoms.
  • As the disease progresses, symptoms may include fatigue, weakness, loss of appetite, easy bruising or bleeding, jaundice, itching, swelling in the legs and abdomen, and weight loss.
  • Advanced cirrhosis can lead to liver failure and life-threatening complications.

Diagnostic Evaluation:

  • Blood tests to evaluate liver function, including liver enzyme and liver function tests.
  • Imaging tests, such as ultrasound, CT scans, and MRI, to visualize liver damage.
  • Liver biopsy to assess the degree of liver damage and confirm the diagnosis.
  • Screening for liver cancer, which can develop in people with cirrhosis.

Treatment and Management:

  • The treatment aims to slow the progression of scar tissue in the liver and manage symptoms and complications.
  • Avoiding alcohol and losing weight for NAFLD or NASH.
  • Medications to control hepatitis, immune system disorders, and other conditions.
  • Treatment for complications such as portal hypertension, varices, ascites, and encephalopathy. This may include diuretics, beta-blockers, and lactulose.
  • Liver transplant may be considered in cases of liver failure.

Complications:

  • Portal hypertension, leading to varices and an increased risk of bleeding.
  • Ascites and edema.
  • Hepatic encephalopathy, leading to confusion and coma.
  • Increased risk of liver cancer.

Prevention and Patient Education:

  • Reducing the risk of cirrhosis is possible by avoiding excessive alcohol consumption, maintaining a healthy weight, and managing risk factors for viral hepatitis.
  • Regular medical follow-up for individuals with liver diseases or at risk of cirrhosis.
  • Vaccinations against hepatitis A and B for at-risk individuals.
