  2. Irrigation: A small amount of warm or cold water or air is introduced into the ear canal of one ear at a time. This is done using a special device called a caloric irrigator.
  3. Stimulation duration: The warm or cold stimulus is delivered for about 30 to 60 seconds to each ear separately.
  4. Observation: The patient's eye movements are observed and recorded during the stimulation and recovery periods.
  5. Repeat: The procedure is then repeated for the other ear.

Interpretation of the Caloric Reflex Test:

  • Nystagmus direction: The caloric reflex test induces nystagmus, which can be horizontal, vertical, or torsional in nature. The direction and intensity of the eye movements are noted.
  • Asymmetry: A significant difference in nystagmus response between the ears may suggest an underlying vestibular pathology.
  • Warm and cold responses: The response to warm and cold stimulation is assessed separately. Normally, warm water or air induces a slow phase of nystagmus towards the irrigated ear, while cold water or air elicits nystagmus away from the irrigated ear.

Clinical Applications of the Caloric Reflex Test:

  • Diagnosis of vestibular disorders: Abnormal responses in the caloric reflex test can aid in diagnosing conditions such as vestibular neuritis, Meniere's disease, or benign paroxysmal positional vertigo (BPPV).
