Procedure:

The procedure for bronchoalveolar lavage typically involves the following steps:

  1. The patient is usually sedated to minimize discomfort during the procedure.
  2. A bronchoscope, a thin, flexible tube with a light and camera at the end, is inserted through the mouth or nose and into the airways.
  3. A small amount of sterile saline solution is instilled into a specific area of the lung.
  4. The fluid is then aspirated back through the bronchoscope and collected in a sterile container for analysis.

Risks:

Bronchoalveolar lavage is generally a safe procedure, but it does carry some risks, such as:

  • Bleeding, particularly in patients who have a low platelet count or who are taking blood thinning medications.
  • Infection, although this is rare.
  • Pneumothorax, which is the accumulation of air in the space between the lung and chest wall. This occurs in less than 1% of patients and is usually self-limited.
  • Respiratory distress, which may occur in patients with underlying lung disease.

Interpretation:

The fluid collected during bronchoalveolar lavage is analyzed for various cellular, biochemical, and microbiologic parameters. The results can provide important information about the underlying cause of the patient's respiratory symptoms. Some of the parameters that are analyzed include:

  • Total cell count, which can help identify whether the patient has an inflammatory or infectious process.
  • Differential cell count, which can help identify the type of inflammatory cells present.
  • Microbiologic cultures, which can help identify any bacteria, viruses, or fungi that may be causing the patient's respiratory symptoms.
  • Cytology, which can help identify any malignant cells that may be present.
