Etiology:

  • Pulmonary Barotrauma: Can lead to serious conditions such as pneumothorax, mediastinal emphysema, and arterial gas embolism, presenting with chest pain, breathing difficulties, and potentially life-threatening cardiovascular symptoms.

Diagnosis:

  • Clinical Assessment: Based on the history of recent pressure changes and symptoms.
  • Physical Examination: Inspection and palpation of the affected areas, with particular attention to the ears and sinuses.
  • Audiometry: May be performed if hearing loss is reported.
  • Imaging: Chest X-ray or CT scan if pulmonary barotrauma is suspected.

Management:

  • Preventive Measures: Education on techniques for pressure equalization, such as the Valsalva maneuver or the use of decongestants before diving or flying.
  • Medical Treatment: Decongestants and nasal steroids for mild cases of ear and sinus barotrauma.
  • Analgesia: Pain management with over-the-counter pain relievers.
  • Dive Planning: For divers, ensuring proper ascent rates and avoiding diving with respiratory infections or congestion.
  • Surgical Intervention: In severe or chronic cases, procedures such as tympanostomy tube insertion may be necessary.

Complications:

  • Chronic Dysfunction: Repeated episodes can lead to chronic pain and hearing or sinus problems.
  • Hearing Loss: Permanent damage to the ear structures can occur in severe cases.
  • Life-Threatening Conditions: Pulmonary barotrauma requires immediate medical attention to address potential complications like arterial gas embolism.

Prognosis:

  • Generally Favorable: Most cases of barotrauma resolve with conservative measures and avoidance of further pressure changes.
  • Monitoring: Some individuals may require follow-up to ensure complete resolution, especially if initial symptoms were severe.
