Definition:

  • The presence of a fishy odor when a potassium hydroxide (KOH) solution is added to the discharge.
  • The presence of vaginal pH greater than 4.5.
  • The presence of "clue cells," which are vaginal epithelial cells covered with bacteria.

Procedure:

The Amsel criteria can be performed in a healthcare provider's office or clinic. The procedure involves collecting a vaginal swab and performing the following tests:

  • Visual examination of the discharge to assess its color and consistency.
  • Addition of a potassium hydroxide (KOH) solution to the discharge to assess for the presence of a fishy odor.
  • Measurement of vaginal pH using a pH test strip.
  • Microscopic examination of the discharge to assess for the presence of clue cells.

Interpretation:

To diagnose bacterial vaginosis using the Amsel criteria, three of the four criteria must be met. If an individual meets three or more of the criteria, they are diagnosed with bacterial vaginosis.

Limitations:

The Amsel criteria is not a perfect diagnostic tool and may not be accurate in all cases. Other factors may contribute to the presence of the criteria, such as the use of antibiotics or hormonal contraceptives.
