Adenosine's efficacy may be limited by the rapid ventricular rates, structural heart disease, and refractory nature of many ventricular tachycardias.

Limitations and Considerations:

Several limitations and considerations should be taken into account when considering adenosine for ventricular tachycardia:

  • Hemodynamic Instability: In hemodynamically unstable patients with VT, adenosine administration may exacerbate hypotension or worsen cardiac output, necessitating cautious use.
  • Underlying Etiology: Adenosine is more likely to be effective in terminating monomorphic VT with a narrow QRS complex compared to polymorphic VT or VT associated with structural heart disease.
  • Alternative Therapies: In the acute management of ventricular tachycardia, interventions such as electrical cardioversion, antiarrhythmic medications (e.g., amiodarone, lidocaine), or catheter ablation are often preferred over adenosine due to their established efficacy and safety profiles.

Conclusion:

Adenosine's role in ventricular tachycardia management is limited and remains controversial. While adenosine may transiently suppress certain forms of ventricular tachycardia, its efficacy is variable, and alternative treatment modalities are generally preferred in clinical practice. Further research is warranted to elucidate the specific subtypes of ventricular tachycardia that may respond to adenosine and to optimize its use in select patient populations.
