Introduction:

Autonomic dysreflexia is a potentially life-threatening condition that can occur in individuals with spinal cord injuries at the level of T6 or higher. It is caused by an exaggerated response of the autonomic nervous system to a noxious stimulus below the level of injury. In this article, we will discuss the causes, symptoms, and treatment options for autonomic dysreflexia.

Causes:

The most common cause of autonomic dysreflexia is a noxious stimulus, such as a bladder or bowel distension, pressure ulcers, or ingrown toenails, below the level of the spinal cord injury. Other potential triggers include skin irritation, tight clothing, or even a change in room temperature. In rare cases, autonomic dysreflexia can occur spontaneously without any apparent trigger.

Symptoms:

Symptoms of autonomic dysreflexia can include:

  • Severe headache
  • Profuse sweating above the level of injury
  • Goosebumps below the level of injury
  • Flushing or redness of the skin above the level of injury
  • Nasal congestion
  • Elevated blood pressure
  • Slow heart rate
  • Anxiety or restlessness
  • Blurred vision

If left untreated, autonomic dysreflexia can lead to seizures, stroke, or even death.

Treatment:

The first step in treating autonomic dysreflexia is to identify and remove the noxious stimulus. This may involve emptying the bladder or bowels, changing clothing or position, or treating pressure ulcers or ingrown toenails. If the blood pressure remains elevated despite removing the trigger, medications may be necessary to lower it. Medications such as nifedipine or hydralazine may be used to dilate blood vessels and lower blood pressure. In severe cases, hospitalization may be necessary for close monitoring and further management.

Conclusion:

Autonomic dysreflexia is a potentially life-threatening condition that can occur in individuals with spinal cord injuries. It is important to be aware of the potential triggers and symptoms of autonomic dysreflexia to prevent and manage this condition. If you suspect you or someone you know has autonomic dysreflexia, seek immediate medical attention.
