Causes:

ABO typing discrepancies may occur due to various reasons, including:

  • Rouleaux formation, which is the aggregation of red blood cells due to the presence of plasma proteins
  • Unexpected antibodies in the serum, such as cold agglutinins or alloantibodies
  • Transfusion reactions

Types:

There are two main types of ABO typing discrepancies:

  1. Type I discrepancies: This type of discrepancy occurs due to technical errors, such as mixing up the samples or mislabeling the tubes. Type I discrepancies are easily preventable by following proper blood typing protocols and ensuring quality control measures.
  2. Type II discrepancies: Type II discrepancies occur due to biological factors, such as genetic variations or unexpected antibodies in the serum. Type II discrepancies can be further categorized into subtypes based on the cause:
    • Subtype A: Occurs due to the presence of an unexpected alloantibody in the serum that reacts with the red blood cells.
    • Subtype B: Occurs due to genetic variations in the ABO genes that produce weak or atypical antigens.
    • Subtype C: Occurs due to the presence of unexpected cold agglutinins that cause red blood cells to clump together.
    • Subtype D: Occurs due to the presence of rouleaux formation that can mimic the agglutination of red blood cells.

Diagnosis:

Diagnosis of ABO typing discrepancies involves a thorough investigation of the cause of the discrepancy. The following tests may be performed to determine the cause:

  • Repeat ABO typing
  • Antibody screening
  • Direct antiglobulin test (DAT)
  • Elution test to identify the presence of unexpected antibodies
  • Genetic testing to identify genetic variations in the ABO genes

Management:

The management of ABO typing discrepancies depends on the type and severity of the discrepancy. The following interventions may be considered:

  • Type I discrepancies: These discrepancies can be resolved by repeating the blood typing procedure and ensuring proper quality control measures are in place.
  • Type II discrepancies: The management of type II discrepancies involves further investigation to determine the cause of the discrepancy. 
    • The following interventions may be considered:
      • Blood typing with alternative methods, such as monoclonal reagents or saliva testing.
      • Cross-matching with the intended blood product to identify any incompatibility.
      • Administration of compatible blood products, such as washed red blood cells or plasma, to avoid transfusion reactions.
      • Consultation with a transfusion medicine specialist or hematologist for further management.
