What is Urothelial Proliferation of Uncertain Malignant Potential? (Definition/Background Information)

  • Urothelial Proliferation of Uncertain Malignant Potential (UPUMP) is a recently described and rare subtype of noninvasive lesion of the urothelium (the urothelial cells that line the urothelial tract). This premalignant lesion can be seen sporadically (de novo) or concurrently with preexistent bladder neoplasms (typically identified during screening cystoscopies)
  • Urothelial Proliferation of Uncertain Malignant Potential is diagnosed under a microscope, on examination of the cancer cells by a pathologist. Earlier, lesions of this nature were categorized under flat and papillary urothelial hyperplasia; these terms are no longer recommended by the WHO classification (2016). The classification of the tumor is important, as it may dictate the type of treatments to be provided and the overall outcomes
  • The exact cause of development of Urothelial Proliferation of Uncertain Malignant Potential is presently unknown; certain genetic alterations have been observed in the tumor. UPUMP is observed in a broad age group (22 to 89 years); most cases are observed during middle and older age (mean 65-67 years). Between 75-80% of the cases are observed in men
  • In many cases, no significant signs and symptoms of Urothelial Proliferation of Uncertain Malignant Potential, such as urinary abnormalities, are noted. Blood in urine (hematuria) is observed in 30-60% of the individuals. A diagnosis of UPUMP is generally difficult and challenging. The treatment may include surgery (total excision) and other modalities, based on the healthcare provider’s assessment
  • According to medical literature, the rate of progression of de novo UPUMP to a higher grade tumor and malignancy respectively is 25% and 17%; while progression rates of UPUMP in the setting of urinary bladder neoplasia is nearly 59% and 40%. The malignancies that may develop include low-grade or high-grade (noninvasive) papillary urothelial carcinoma, urothelial carcinoma in situ, and invasive urothelial carcinoma
