What is Headache Attributed to Unruptured Saccular Aneurysm? (Definition/Background Information)

  • Headache Attributed to Unruptured Saccular Aneurysm is a headache disorder that occurs secondary to an unruptured saccular aneurysm, meaning that the aneurysm has not ruptured, and there is no bleeding
  • A saccular aneurysm of the brain is among the most common intracranial aneurysms noted. An aneurysm is a blood-filled bulge in the artery caused by weakening of the arterial wall
  • The headache that develops has a sudden onset and presents like a thunderclap. It is known to resolve after treatment of the aneurysm

The criteria for diagnosis of Headache Attributed to Unruptured Saccular Aneurysm as outlined by the International Headache Society (IHS) is given below:

  • An unruptured saccular aneurysm has been diagnosed
  • Any new headache fulfilling the criterion below

Evidence of causation demonstrated by the following:

  • Headache has developed in close temporal relation to other symptoms and/or clinical signs of unruptured saccular aneurysm, or has led to its diagnosis
  • Either or both of the following:
    • Headache has significantly worsened in parallel with other symptoms or clinical or radiological signs of growth of the saccular aneurysm
    • Headache has resolved after treatment of the saccular aneurysm
  • Either or both of the following:
    • Headache has sudden or thunderclap onset
    • Headache is associated with a painful third nerve palsy

Not better accounted for by another ICHD-3 diagnosis.
