What is Headache Attributed to Non-Traumatic Intracerebral Hemorrhage? (Definition/Background Information)

  • Headache Attributed to Non-Traumatic Intracerebral Hemorrhage is a headache disorder that occurs secondary to non-traumatic intracerebral hemorrhage. Apart from the often sudden and intense headaches, there may be other associated neurological signs and symptoms noted
  • Non-traumatic intracerebral hemorrhage (ICH) may described as bleeding in the brain that develops from a ruptured blood vessel, in the absence of any injury/trauma to the brain. It is a medical emergency that is associated with a high (early) mortality rate
  • The neurological signs and symptoms of the condition may include paralysis of the speaking and swallowing difficulties, paralysis of the face or limbs, vision impairment, loss of balance, confusion, dizziness, loss of consciousness, including coma. The headache is often severe, and occasionally, may be a prominent presentation of non-traumatic intracerebral hemorrhage

The criteria for diagnosis of Headache Attributed to Non-Traumatic Intracerebral Hemorrhage as outlined by the International Headache Society (IHS) is given below:

  • Intracerebral hemorrhage (ICH) in the absence of head trauma has been diagnosed
  • Headache fulfilling the criterion below

Evidence of causation demonstrated by at least two of the following:

  • Headache has developed in temporal relation to other symptoms and/or clinical signs of ICH, or has led to the diagnosis of ICH
  • Headache has significantly improved in parallel with stabilization/improvement of other symptoms or clinical or radiological signs of ICH
  • Headache has at least one of the following three characteristics:
    • Sudden or thunderclap onset
    • Maximal on the day of its onset
    • Localized in accordance with the site of the hemorrhage

Either of the following:

  • Headache has resolved within 3 months (of stabilization/treatment or spontaneously)
  • Headache has not yet resolved but 3 months have not yet passed

Not better accounted for by another ICHD-3 diagnosis.
