What is Dialysis Headache? (Definition/Background Information)

  • Dialysis Headache is a headache disorder that occurs secondary to hemodialysis, which is a process of artificial blood purification (removal of wastes) when the kidneys remain dysfunctional. The headache, which may be of any type, starts during hemodialysis, to resolve spontaneously by its own within 3 days following completion of the dialysis session
  • According to research, headaches are more common in individuals with low blood pressure (hypotension) and dialysis disequilibrium syndrome (DDS), a rare condition of neurologic deterioration in individuals undergoing long-term dialysis. Severe cases of DDS can lead to moderate reduction in one’s alertness, stupor, seizures, gradually progressing to coma and death

The criteria for diagnosis of Dialysis Headache as outlined by the International Headache Society (IHS) is given below:

  • The patient is on hemodialysis
  • At least three episodes of acute headache fulfilling the criterion below

Evidence of causation demonstrated by at least two of the following:

  • Each headache has developed during a session of hemodialysis
  • Either or both of the following:
    • Each headache has worsened during the dialysis session
    • Each headache has resolved within 72 hours after the end of the dialysis session
  • Headache episodes cease altogether after successful kidney transplantation and termination of hemodialysis

Not better accounted for by another ICHD-3 diagnosis.

Differential diagnosis: It is important to exclude caffeine-withdrawal headache prior to establishing a diagnosis of Dialysis Headache, in individuals who consume excess quantities of beverages, such as coffee, or other caffeine products.
