What is Cerebrospinal Fluid Fistula Headache? (Definition/Background Information)

  • Cerebrospinal Fluid (CSF) Fistula Headache is a headache disorder that occurs secondary to certain procedures or traumatic brain injuries (skull fractures) resulting in persistent internal leakage of the cerebrospinal fluid causing reduced CSF pressure in the brain and spinal cord. The condition resolves on managing (sealing) the CSF leakage suitably. A fistula is defined as an abnormal passage between conduit that connects two spaces
  • Any decrease in CSF pressure can result in neurological signs and symptoms, including commonly orthostatic headaches. Individuals with orthostatic headaches may experience a significant increase in intensity of the headaches on sitting or standing up from a lying or seated position respectively. In many, the condition may improve on lying down horizontally

The criteria for diagnosis of Cerebrospinal Fluid (CSF) Fistula Headache as outlined by the International Headache Society (IHS) is given below:

  • A procedure has been performed, or trauma has occurred, known sometimes to cause persistent CSF leakage (CSF fistula)
  • Either or both of the following:
    • Low cerebrospinal fluid (CSF) pressure (less than 60 mm CSF)
    • Evidence of CSF leakage on imaging
  • Any headache fulfilling criterion below
  • Headache has developed in temporal relation to the procedure or trauma
  • Not better accounted for by another ICHD-3 diagnosis
